ABSTRACT

Abstract Background: Pleural empyema secondary to a ruptured amoebic liver abscess is a rare complication in the pediatric population. Case report: We report the case of a 13-year-old male with right flank abdominal pain, productive cough with foul-smelling sputum, fever, and respiratory distress. Physical examination revealed breathlessness, decreased vesicular murmur in the right hemithorax, abdominal distension, hepatomegaly, and lower limb edema. Laboratory tests revealed mild anemia, leukocytosis without eosinophilia, elevated alkaline phosphatase, hypoalbuminemia, and positive immunoglobulin G antibodies against Entamoeba histolytica in pleural fluid. He required a chest tube and treatment with metronidazole. After 2 months of follow-up, the abscesses disappeared, and the empyema decreased. Conclusions: Massive pleural empyema secondary to a ruptured liver abscess is a rare complication. The epidemiological link associated with the symptoms and serological tests can help in the diagnosis.


Resumen Introducción: El empiema pleural secundario a ruptura de absceso amebiano hepático es una complicación poco frecuente en la población pediátrica. Caso clínico: Se reporta el caso de un paciente de sexo masculino de 13 años que presentó dolor abdominal en flanco derecho, tos productiva con esputo de mal olor, fiebre y dificultad respiratoria. Al examen físico se encontró amplexación y murmullo vesicular disminuido en hemitórax derecho, distensión abdominal, hepatomegalia y edema de miembros inferiores. Los resultados del laboratorio evidenciaron anemia leve, leucocitosis sin eosinofilia, elevación de fosfatasa alcalina, hipoalbuminemia y anticuerpos IgG contra Entamoeba histolytica positivo en líquido pleural. Requirió tubo de drenaje torácico y tratamiento con metronidazol. A los dos meses de seguimiento los abscesos desaparecieron y el empiema disminuyó. Conclusiones: El empiema pleural masivo secundario a ruptura de absceso hepático es una complicación poco frecuente. El nexo epidemiológico asociado con la sintomatología y pruebas serológicas pueden ser de ayuda en el diagnóstico.

ABSTRACT

RESUMEN Se reporta el caso de un varón de 62 años quien presentó tos y dolor abdominal por dos semanas, con hallazgos tomográficos pulmonares compatibles con probable infección por SARS-CoV-2, que recibió tempranamente corticoterapia a dosis altas y de manera ambulatoria. El paciente presentó evolución tórpida, fue hospitalizado y falleció en el posoperatorio inmediato por un cuadro de abdomen agudo quirúrgico. El estudio anatomopatológico mostró estructuras parasitarias con características compatibles con amebas, como causa de una colitis aguda fulminante complicada, con perforación múltiple y peritonitis aguda. La colitis aguda fulminante por amebiasis intestinal en el contexto de un paciente con probable COVID-19 no había sido reportada en el Perú. Resulta importante resaltar la presentación fulminante con desenlace fatal de esta infección parasitaria prevalente en nuestro medio, en el contexto del uso de corticoides ante una probable infección pulmonar por COVID-19.


ABSTRACT We report the case of a 62-year-old male with cough and abdominal pain for two weeks, associated with pulmonary tomographic findings compatible with probable infection by SARS-CoV-2, and who received high doses of early corticotherapy as an outpatient. The patient showed clinical deterioration, was hospitalized and died in the immediate postoperative period due to acute surgical abdomen. The anatomopathological study showed parasitic structures with characteristics compatible with amebae, which was pointed to as the cause of a complicated acute fulminant colitis, with multiple perforations and acute peritonitis. Acute fulminant colitis due to intestinal amebiasis in the context of a patient with probable COVID-19 had not been previously reported in Peru. It is important to highlight the fulminant presentation with fatal outcome of this prevalent parasitic infection, in the context of corticosteroids use in a probable SARS-CoV-2 pulmonary infection.

Introducción: Naegleria fowleri, Acanthamoeba spp. y Balamuthia mandrillaris son amebas de vida libre que producen daños sustanciales del sistema nervioso central y cuyo diagnóstico premortem es poco frecuente. Objetivo: Proveer una visión general de los aspectos clínico-epidemiológicos y las posibilidades diagnósticas más eficaces en la meningoencefalitis amebiana primaria y la encefalitis granulomatosa amebiana, así como las experiencias de tratamiento expuestas en la literatura reciente. Métodos: Se realizó una revisión bibliográfica sobre las amebas de vida libre. Se analizó la información obtenida de artículos científicos en la base de datos Google Scholar®, PubMed y las citas relacionadas por el programa en PubMed Central. Análisis y síntesis de la información: En primer término del análisis de la meningoencefalitis amebiana primaria y la encefalitis granulomatosa amebiana, resalta la diferenciación de sus características clínicas y epidemiológicas, también que el líquido cefalorraquídeo presenta gran relevancia para el diagnóstico de la meningoencefalitis amebiana primaria. Sin embargo, para los casos presuntivos de la encefalitis granulomatosa amebiana el examen del líquido cefalorraquídeo depende de la valoración riesgo-beneficio y se ha obtenido mayor positividad con el diagnóstico histopatológico de biopsias premortem. En general, se acrecienta la implementación de la investigación del ADN por diferentes métodos que brindan certeza de cada una de las especies de AVL causantes de enfermedad neurológica. El aislamiento en cultivo confirmatorio de N. fowleri, Acanthamoeba spp. y B. mandrillaris presenta diferencias en la factibilidad de crecimiento en diversos medios de acuerdo con la especie y limitaciones adicionales. Conclusiones: La mortalidad asociada con las infecciones del sistema nervioso central por amebas de vida libre permanece alta, aunque varios estudios brindan experiencias útiles a partir de los casos de pacientes que han sobrevivido. Resulta importante tener en cuenta que el diagnóstico rápido de la infección es clave para un tratamiento exitoso(AU)


Introduction: Naegleria fowleri, Acanthamoeba spp. and Balamuthia mandrillaris are free-living amoebae of infrequent premortem diagnosis which cause substantial damage to the central nervous system. Objective: To provide an overview of the clinical-epidemiological aspects and the most effective diagnostic possibilities in primary amebic meningoencephalitis and granulomatous amebic encephalitis, as well as their treatment experiences in recent publications.. Methods: A bibliographic review was conducted about free-living amoebae. An analysis was performed of data obtained from scientific papers published in the databases Google Scholar and PubMed, and the citations listed by the PubMed Central program. Data analysis and synthesis: As principal term of the analysis of primary amebic meningoencephalitis and amebic granulomatous encephalitis, the differentiation of their clinical and epidemiological characteristics stands out, also that cerebrospinal fluid is highly relevant for the diagnosis of primary amebic meningoencephalitis. However, for presumptive cases of amoebic granulomatous encephalitis, the examination of cerebrospinal fluid depends on the risk-benefit assessment and greater positivity has been obtained with the histopathological diagnosis of premortem biopsies. In general, the implementation of DNA research by different methods provided accurate information about each one of the free-living amoeba species that cause neurological disease. Confirmatory culture isolation of N. fowleri, Acanthamoeba spp. and B. mandrillaris revealed growth feasibility differences between diverse media depending on the species and additional limitations. Conclusions: Mortality associated to central nervous system infections caused by free-living amoebae remains high. Studies describing cases of patients who have survived constitute useful material. It is important to bear in mind that fast diagnosis of the disease is crucial to treatment success(AU)

ABSTRACT

Although infrequent in the diagnostics areas, liver abscesses are of significant relevance, which is related to the high morbidity and mortality figures they can cause if they are not detected and treated in time. Although multiple operational classifications are described, the one most recognized for its usefulness is that related to its causative agent (pyogenic or bacterial infection, hydatid origin or secondary to infected hydatidosis, and amoebic or by invasive amebiasis). The objective of this article was to generate a study report regarding the morphological characteristics of liver abscesses, characterizing them according to their etiology, as well as describing their study and the latest recommended treatments.


Los abscesos hepáticos son entidades nosológicas de baja frecuencia, pero de significativa relevancia, la que se relacionada con las elevadas cifras de morbilidad y mortalidad que pueden causar si no son detectados y tratados a tiempo. Si bien se describen múltiples clasificaciones operacionales, aquella más reconocida por su utilidad es la relacionada a su agente causal (piógenos o bacterianos, hidatídicos o secundarios a hidatidosis infectada, y amebianos o por amebiasis invasora). El objetivo de este artículo fue generar un documento de estudio respecto de las características morfológicas de los abscesos hepáticos, caracterizándolos de acuerdo con su etiología, así como describir su estudio y tratamiento recomendado de acuerdo con el estado del arte.

ABSTRACT

A diarreia crônica, caracterizada pela presença de mais de três dejeções de consistência pastosa durante pelo menos quatro semanas, é frequentemente encontrada na prática clínica. Na diarreia crônica, a probabilidade de uma etiologia infecciosa é baixa, sendo as causas funcionais, inflamatórias, osmóticas ou secretórias mais comuns. A amebíase intestinal é uma causa de diarreia crônica, causada pelo protozoário Entamoeba histolytica, sendo comum e prevalente em países em desenvolvimento. Pode ter várias formas de apresentação, sendo na maioria dos casos assintomática. Apresenta-se o caso clínico de um paciente de 27 anos, sexo masculino, raça caucasiana, homem que pratica sexo com homens (HSH), com diarreia crônica com início há cerca de seis anos. O exame objetivo não revelava alterações. Realizou exame parasitológico de fezes, com isolamento de quistos de Entamoeba histolytica. Foi medicado com metronidazol e paromomicina com resolução clínica do quadro. É importante para o Médico de Família equacionar esta etiologia na investigação de pacientes com diarreia, para um correto e atempado diagnóstico e tratamento, de modo a evitar exames desnecessários, possíveis complicações, a transmissão do agente e um grave problema de saúde pública.


Chronic diarrhea, characterized by the presence of more than three loose stools for at least four weeks, is often found in clinical practice. In chronic diarrhea, the probability of an infectious etiology is low, having usually a functional, inflammatory, osmotic or secretory origin. Intestinal amebiasis is a cause of chronic diarrhea, caused by the protozoan Entamoeba histolytica, common and prevalent in developing countries. It can have different presentations but most patients are asymptomatic. We present a 27-year-old Caucasian male, man who have sex with men (MSM), with chronic diarrhea starting six years ago. The physical exam was normal. A parasitological stool examination was performed with isolation of cysts of Entamoeba histolytica. He was treated with metronidazole and paromomycin with clinical resolution. It is important for General Practice to address this etiology in the investigation of patients with diarrhea for a correct and timely diagnosis and treatment, to avoid unnecessary testing, possible complications, transmission of the agent and a serious public health problem.


La diarrea crónica, caracterizada por la presencia de más de tres heces de consistencia blanda durante al menos cuatro semanas, es frecuentemente encontrada en la práctica clínica. En la diarrea crónica, la probabilidad de una etiología infecciosa es baja, siendo las causas funcionales, inflamatorias, osmóticas o secretoras más comunes. La amebiasis intestinal es una causa de diarrea crónica, causada por el protozoario Entamoeba histolytica, siendo común y prevalente en los países en desarrollo. Puede tener varias formas de presentación, siendo en la mayoría de los casos asintomática. Presentamos un paciente de 27 años, sexo masculino, caucásico, hombre que tiene sexo con hombres (HSH), con diarrea crónica con inicio hace cerca de seis años. El examen objetivo no reveló alteraciones. Realizó un examen parasitológico de heces, con aislamiento de quistes de Entamoeba histolytica. Fue medicado con metronidazol y paromomicina con resolución clínica del cuadro. Es importante para el médico de familia considerar esta etiología en la investigación de pacientes con diarrea, para un correcto y oportuno diagnóstico y tratamiento, de modo a evitar, exámenes innecesarios, posibles complicaciones, la transmisión del agente y un grave problema de salud pública.

ABSTRACT

Primary amebic encephalitis (PAM) is a devastating central nervous system infection caused by Naegleria fowleri, a free-living amoeba, which can survive in soil and warm fresh water. Here, a 43-year-old healthy male was exposed to warm freshwater 5 days before the symptom onset. He rapidly developed severe cerebral edema before the diagnosis of PAM and was treated with intravenous conventional amphotericin B while died of terminal cerebral hernia finally. Comparing the patients with PAM who has similar clinical symptoms to those with other common types of meningoencephalitis, this infection is probably curable if treated early and aggressively. PAM should be considered in the differential diagnosis of purulent meningoencephalitis, especially in patients with recent freshwater-related activities during the hot season.

Amebiasis, a parasitic infection caused by Entamoeba histolytica, used to be a prevalent protozoan disease in Korea, however, with an improving sanitary system, it has been among very uncommon etiology of liver abscess. A recent report suggested that ALA is an emerging parasitic infection in human immunodeficiency virus (HIV)-infected patients even in areas where the disease is not endemic and recommended HIV screening in patients in areas where ALA is not endemic, particularly those without history of travel to a disease-endemic area. We report on two patients who were admitted for treatment of ALA and then diagnosed as HIV infection. We also reviewed the etiology and characteristics of ALA in our hospital during the last 5 years.

Background: Intestinal amebiasis is one of the important differential diagnoses of Inflammatory Bowel Disorders in areas where it is highly prevalent. Aim: Studies comparing the clinical, endoscopic and histological features of these disorders have never been done, so we undertook this study. Materials and Methods: A retrospective study comparing mucosal biopsies of 14 consecutive cases of intestinal amebiasis with 14 cases of Ulcerative colitis and 12 cases of Crohn’s disease. A total of 65 biopsies from patients with amebiasis, 56 biopsies from patients with Crohn’s disease and 65 biopsies of patients with Ulcerative colitis were reviewed. Results and Conclusions: Discrete small ulcers less than 2 cm in diameter in the cecum or rectosigmoid, with intervening normal mucosa, were the most common finding on endoscopy in patients with amebiasis. On histology, necrotic material admixed with mucin, proteinaceous exudate and blood clot lining ulcers, significant surface epithelial changes such as shortening and tufting adjacent to sites of ulceration, mild chronic inflammation extending into the deep mucosa and mild architectural alteration were features of amebiasis. Trophozoite forms of ameba were seen in the necrotic material lining sites of ulceration or lying separately, as well as over intact mucosa. Necrotic material lining ulcers was less common in IBD, but chronic inflammation, crypt abscess formation and architectural alteration were more severe.

All organs of the immune system are innervated and almost all neurotransmitter receptors are present on immune cells. We studied the effects of sympathetic innervation in the development of amebic liver abscess (ALA) in rats. Our results showed that lack of sympathetic innervation promote a decrease in size of ALA. We found scarce amoebas, increased the number of neutrophils and a few collagen fibers surrounding the abscess, meanwhile in control group, we observed abscesses areas with typical necrosis including trophozoites and neutrophils. Macrophages were differentially distributed surrounding abscess area in control and vehicle groups, but equally located in and outside of the abscesses in sympathectomized rat. No significant differences were observed on NK cells in analysed groups. In cytokines quantification studies, we observed down-expression of IFN-g and TNF-a, moreover, we found overexpression of IL-10 in sympathectomized and ALA group. In conclusion, our results suggest that elimination of sympathetic nerve fibers in a model rat of amebic liver abscess induces reduction of the innate immune response and presence of amebas through the liver at seven days post-inoculation.


Todos los órganos del sistema inmune están inervados y casi todos los receptores para neurotransmisores están presentes en las células de la respuesta inmune. Nosotros estudiamos el efecto de la inervación simpática en el desarrollo del Absceso Hepático Amebiano (AHA) en ratas. Nuestros resultados muestran que la inervación simpática promueve una disminución en el tamaño del AHA. Nosotros encontramos áreas fibróticas bien definidas con algunas amibas, mayor número de neutrófilos y pocas fibras de colágena rodeando el área de daño, mientras que en el grupo control, nosotros observamos áreas con necrosis, trofozoítos y pocos neutrófilos en el área fibrótica. Los macrófagos se observaron distribuidos en el área fibrótica en los animales simpatectomizados, mientras que en los controles encontramos a los macrófagos distribuidos en la periferia del absceso. No se encontró diferencia significativa en la distribución y cantidad de células NK. En el estudio de citocinas nosotros observamos una disminución de IFN-g y TNF-a y un incremento de IL-10 en animales simpatectomizados. En conclusión, nuestros resultados sugieren que la eliminación de las fibras del sistema nervioso simpático en el modelo de AHA en rata, reduce la respuesta inmune innata y persisten amebas en el tejido dañados a los 7 días post-inoculación.

ABSTRACT

La infección por Entamoeba histolytica sigue siendo un problema mayor de salud pública en los países en desarrollo como Colombia. Aunque en la gran mayoría de casos se presenta como una disentería o un absceso hepático amebiano, existen casos donde la evolución genera una verdadera masa inflamatoria en el colon llamada ameboma. Estos amebomas son fácilmente confundidos con una lesión neoplásica y, de no tener un gran nivel de sospecha clínica o endoscópica, terminan en una resección de colon con un diagnóstico posoperatorio dado por la presencia de trofozoítos sin malignidad. Sin embargo, cuando el diagnóstico se hace de manera preoperatoria, podría haber resolución del cuadro sin necesidad de cirugía. Exponemos el caso de una rara presentación de ameboma en el recto asociado con un absceso hepático perforado, en el cual se hizo un diagnóstico preoperatorio, pero dada la ruptura de la lesión en el hígado, requirió de una intervención quirúrgica.


Entamoeba histolytica infections continue to be a major public health problem in developing countries such as Colombia. Although it most often presents as dysentery or amebic liver abscess, there are cases in which a true inflammatory mass develops in the colon. This is called an ameboma. These amebomas are easily confused with neoplastic lesions so that in the absence of a high level of clinical and endoscopic suspicion, a colon resection may be performed with the postoperative diagnosis given by the presence of trophozoites without malignancy. However, when the diagnosis is made preoperatively symptoms can be without surgery. We report a rare presentation of ameboma in the rectum associated with a perforated liver abscess. This was diagnosed prior to surgey, but given the liver injury, required surgery.

ABSTRACT

In amebic liver abscess, communication between liver abscess and intrahepatic bile ducts is an uncommon cause of bile leak. This condition can be treated surgically or endoscopically. However, these treatment modalities are related with high morbidity and mortality. A 49-year-old man was diagnosed with amebic liver abscess. Percutaneous drainage was performed due to poor medical response and for the purpose of preventing abscess rupture. Liver abscess-biliary communication was found at follow-up imaging study. He was treated successfully with medical therapy and supportive care without further interventions.

ABSTRACT

O bloqueio do TNF tem tido sucesso no tratamento de algumas doenças reumáticas, como a espondiloartrite. Relatam-se muitas complicações infecciosas com a terapia anti-TNF, principalmente infecções bacterianas, micobacterianas, virais e fúngicas. A Entamoeba histolytica é um protozoário extracelular que causa principalmente colite e abscesso hepático, sendo que a perfuração intestinal é uma complicação rara, com alta mortalidade. O TNF é considerado o principal mediador da imunidade celular contra a amebíase. Inicialmente, é quimiotático para a E. histolytica, potencializando sua adesão ao enterócito por meio da lectina galactose-inibível, e depois ativando os macrófagos para matarem a ameba pela liberação de NO; assim, o bloqueio do TNF poderia ser prejudicial, aumentando a virulência amebiana. Descreve-se o caso de uma mulher de 46 anos com espondiloartrite que apresentou uma perfuração do colo por colite amebiana invasiva durante uso de anti-TNF.


TNF blockade has been successful in the treatment of some rheumatic diseases such as spondyloarthritis. Many infectious complications have been reported with anti-TNF therapy, mainly bacterial, mycobacterial, viral and fungal infections. Entamoeba histolytica is an extracellular protozoan parasite that mainly causes colitis and hepatic abscess; bowel perforation is an uncommon complication with high mortality. TNF is considered the principal mediator of cell immunity against amebiasis. Initially, it is chemotactic to E. histolytica, enhancing its adherence to enterocyte via galactose inhibitable lectin, and then activating macrophages to kill ameba though the release of NO, so that TNF blocking could be harmful, increasing amebic virulence. We describe the case of a 46-year-old woman with spondyloarthritis who presented a colonic perforation due to invasive amebic colitis during anti-TNF use.

ABSTRACT

A falta de saneamento básico e as dificuldades de acesso aos serviços de saúde favorecem o poliparasitismo, contribuindo para retardo no desenvolvimento cognitivo e físico de crianças ou evolução com complicações que colocam em risco sua saúde. Relata-se aqui uma complicação de abscesso hepático em criança de três anos, poliparasitada, proveniente de comunidade ribeirinha, e discute-se sobre as condições de saneamento e acesso à saúde dessa população. Os dados foram obtidos junto à equipe multiprofissional envolvida durante a internação, ao prontuário e familiares. Dados da literatura são compatíveis com o relato ao mostrarem que, apesar das medidas de ampliação do acesso à saúde via fluvial e da melhoria sanitária em algumas comunidades, ainda há locais desassistidos pelo poder público, sendo necessária a realização de mais estudos para o conhecimento do perfil dessa população e a implantação de políticas públicas que visem melhorar a qualidade de vida, com menores custos em longo prazo.


The lack of sanitation and poor access to health services favor polyparasitism, contributing to delayed cognitive and physical development of children or evolution to complications that endanger their health. This study reports a complication of liver abscess in a three-years-old child with polyparasitism from a riverside community, and discusses the sanitation conditions and health care access of this population. During the child's hospitalization, data were obtained from the multidisciplinary team, medical records, and the family. Literature data are consistent with this report showing that, despite the measures taken to increase access to health care, communities underserved by the local government still exist. Further studies to know the profile of this population and the implementation of public policies aiming to improve the quality of life with lower costs in the long term, are still necessary.


La falta de saneamiento básico y las dificultades en el acceso a los servicios de salud favorecen el poliparasitismo, contribuyendo al retraso en el desarrollo cognitivo y físico de los niños, o bien a su evolución, con complicaciones que ponen en peligro su salud. Se presenta aquí una complicación de absceso hepático en una niña de tres años con poliparasitismo desde una comunidad ribereña, y se discute acerca de las condiciones de saneamiento y acceso a los servicios de salud de esta población. Los datos se obtuvieron junto al equipo multidisciplinario participante durante la hospitalización, de los registros médicos y de los familiares. Los datos de la literatura son compatibles con el caso y muestran, que aunque haya mejoras en el acceso a los servicios de salud en algunas comunidades, todavía quedan comunidades desatendidas por el gobierno, lo que exige más estudios para conocer el perfil de esta población y implementar políticas públicas que mejoren la calidad de vida, con menores costos y a largo plazo.

BACKGROUND: Entamoeba histolytica is an important etiologic agent of diarrhea. Globally, it is estimated to infect 40 to 50 million people and cause 40,000 to 100,000 deaths per year. Metronidazole is effective but can cause adverse reactions in certain individuals. In search of alternatives, traditional medicinal plants are being studied. Several plants in Family Simaroubaceae have shown anti-amoebic activity. Quassia amara, a member of this family has not been tested.OBJECTIVE: To determine the effect of Q. amara crude extract on Entamoeba histolytica in vitro.METHODS: Initial testing of 104 µg/ml ethanolic bark extract was performed. Counts were made after 72 hours. Three trials in triplicates were performed.Nine (9) dilutions of extract were then tested (18.8 to 5,00 µg/ml). Test tubes were checked for viable amoeba after 24-hour and 72-hour incubation. Minimum inhibitory concentrations (MIC) were determined for the two incubation periods. At least two trials in triplicates for each dilution were performed. metronidazole served as positive control.RESULTS: At 104 µg/ml incubated for 72 hours, no viable amoeba was obtained and counted. The MIC after 24 hours was 5,000 µg/ml, while the MIC at 72 hours was 37.5 µg/ml.CONCLUSION: Q. amara crude extract has inhibitory effects on E. histolycain vitro.

ABSTRACT

El absceso hepático amebiano (AHA) es la manifestación extraintestinal más frecuente de la amebiasis. El AHA es resultado de la invasión de Entamoeba histolytica al hígado. Es una infección que está asociada a las malas condiciones de saneamiento ambiental y a los deficientes hábitos higiénicos. Es una patología que aún sigue presentado una alta morbilidad. En este artículo se informa un caso clínico y seguimiento de un paciente de 23 años con AHA, quien presenta la sintomatología típica a quien se suministra tratamiento etiológico adecuado. Adicionalmente, se realiza una revisión sobre el tema abordando los aspectos epidemiológicos más relevantes.


Amebic liver abscess (ALA) is the most common extraintestinal manifestation of amebiasis. The AlA is a result of the invasion of Entamoeba histolytica in the liver. It is an infection that is associated with poor environmental sanitation and poor hygiene. It is a disease that is still presented a high morbidity. This article presents a case report and track a patient aged 23 with ALA, who has the typical symptoms who provided adequate etiological treatment is reported. Additionally, a review of the subject is done by addressing the most relevant epidemiological aspects.

BACKGROUND/AIMS: Many parasites induce changes in the lipid profiles of the host. Cholesterol increases the virulence of Entamoeba histolytica in animal models and in vitro culture. This study aimed to determine, in patients with an amebic liver abscess, the correlation between cholesterol and other features, such as the size and number of abscesses, standard hematological and serum chemistry profiles, liver tests, and duration of hospital stay. METHODS: A total of 108 patients with an amebic liver abscess and 140 clinically healthy volunteers were investigated. Cholesterol and triglycerides were measured in the sera. The data from medical observations and laboratory tests were obtained from the clinical records. RESULTS: A total of 93% of patients with an amebic liver abscess showed hypocholesterolemia not related to any of the studied parameters. Liver function tests correlated with the size of the abscess. The most severe cases of amebic liver disease or death were found in patients whose cholesterol levels continued to decrease despite receiving antiamebic treatment and hospital care. CONCLUSIONS: Our results show that the hypocholesterolemia observed in patients with an amebic liver abscess is not related to any of the clinical and laboratory features analyzed. This is the first study relating hypocholesterolemia to severity of hepatic amebiasis.

ABSTRACT

To identify sequences of Entamoeba histolytica associated with the development of amebic liver abscess (ALA) in hamsters, subtractive hybridization of cDNA from E. histolytica HM-1:IMSS under 2 growth conditions was performed: 1) cultured in axenic medium and 2) isolated from experimental ALA in hamsters. For this procedure, 6 sequences were obtained. Of these sequences, the mak16 gene was selected for amplification in 29 cultures of E. histolytica isolated from the feces of 10 patients with intestinal symptoms and 19 asymptomatic patients. Only 5 of the 10 isolates obtained from symptomatic patients developed ALA and amplified the mak16 gene, whereas the 19 isolates from asymptomatic patients did not amplify the mak16 gene nor did they develop ALA. Based on the results of Fisher's exact test (P<0.001), an association was inferred between the presence of the mak16 gene of E. histolytica and the ability to develop ALA in hamsters and with the patient's symptoms (P=0.02). The amplification of the mak16 gene suggests that it is an important gene in E. histolytica because it was present in the isolates from hamsters that developed liver damage.

ABSTRACT

Liver abscess is an unusual but potentially lethal disease. We should be especially cautious in patients diagnosed with biliary pathology or immunosuppression, with right upper quadrant pain, fever or jaundice. The study should include images, cultures and serology for Entamoeba histolytica in certain scenarios. The treatment of pyogenic liver abscess is based on prolonged antibiotic therapy and usually in the drainage of the collection, which can be percutaneous, open or endoscopic. In case of amoebic liver abscesses, drug treatment -in two phases- achieve the parasitic removal at tissue and luminal levels, keeping the drain choice for larger abscesses.


El absceso hepático es una patología poco prevalente pero potencialmente letal. Se debe tener una alta sospecha en pacientes con patología de la vía biliar o inmunosupresión, que presenten dolor en hipocondrio derecho, fiebre o ictericia. El estudio debe incluir imágenes, cultivos y eventualmente serología para Entamoeba histolytica en determinados escenarios. El tratamiento de los abscesos hepáticos piógenos se basa en antibioterapia prolongada y habitualmente en el drenaje de la colección, el cuál puede ser percutáneo, abierto o endoscópico. En el caso de los abscesos hepáticos amebianos el tratamiento farmacológico, en dos fases, logra la eliminación parasitaria a nivel tisular y luminal, reservando el drenaje para los de gran tamaño.

ABSTRACT

The parasite Entamoeba histolytica causes amebic colitis and systemic amebiasis. Among the known amebic factors contributing to pathogenesis are signaling pathways involving heterotrimeric and Ras superfamily G proteins. Here, we review the current knowledge of the roles of heterotrimeric G protein subunits, Ras, Rho and Rab GTPase families in E. histolytica pathogenesis, as well as of their downstream signaling effectors and nucleotide cycle regulators. Heterotrimeric G protein signaling likely modulates amebic motility and attachment to and killing of host cells, in part through activation of an RGS-RhoGEF (regulator of G protein signaling-Rho guanine nucleotide exchange factor) effector. Rho family GTPases, as well as RhoGEFs and Rho effectors (formins and p21-activated kinases) regulate the dynamic actin cytoskeleton of E. histolytica and associated pathogenesis-related cellular processes, such as migration, invasion, phagocytosis and evasion of the host immune response by surface receptor capping. A remarkably large family of 91 Rab GTPases has multiple roles in a complex amebic vesicular trafficking system required for phagocytosis and pinocytosis and secretion of known virulence factors, such as amebapores and cysteine proteases. Although much remains to be discovered, recent studies of G protein signaling in E. histolytica have enhanced our understanding of parasitic pathogenesis and have also highlighted possible targets for pharmacological manipulation.
